Raw Fitness Inc. – Liability Waiver & Release of Claims
IMPORTANT – PLEASE READ CAREFULLY BEFORE SIGNING
By signing this agreement, you acknowledge and accept the risks associated with physical exercise and the use of Raw Fitness Inc.’s facilities, equipment, programs, and services.
1. Assumption of Risk
I, the undersigned member/participant, understand that participation in fitness training, classes, and use of equipment at Raw Fitness Inc. involves risks of injury, illness, or other health conditions. These risks may include, but are not limited to: muscle strains, sprains, falls, equipment malfunction, exposure to contagious illnesses, and other unforeseen accidents. I voluntarily choose to participate with full knowledge of these risks.
2. Release of Liability
In consideration of being permitted to use the facilities, equipment, and services of Raw Fitness Inc., I hereby release, waive, and discharge Raw Fitness Inc., its owners, directors, employees, contractors, agents, and representatives from any and all liability, claims, demands, actions, or causes of action arising out of or related to any injury, illness, disability, death, or property damage that may occur while on the premises, whether caused by negligence or otherwise.
3. Indemnification
I agree to indemnify and hold harmless Raw Fitness Inc. and its representatives from any and all claims, damages, or expenses (including legal fees) that may arise from my use of the facility, equipment, or participation in any program or activity.
4. Medical Clearance
I acknowledge that I should consult with a physician before starting any exercise program. I represent that I am in good physical condition, have no undisclosed medical conditions that would prevent safe participation, and that I am solely responsible for monitoring my own health during participation.
5. Facility Rules
I agree to follow all posted rules, policies, and instructions given by Raw Fitness Inc. staff. I understand that failure to do so may result in suspension or termination of membership without refund.
Binding Effect
This release shall be binding upon me, my heirs, executors, administrators, and assigns. If any portion of this waiver is found unenforceable, the remaining provisions shall remain in full force and effect.
